The pic you are viewing clearly shows as in many other images of Buddha’s head strange little ‘bumps’ and a rear higher elevation of the  head.

Now that my hair is much shorter, as in Buddhist style, I just remembered my consternation and cincern prior to all this business of enlightenment in which all of those little cute bumps where definitely,  positively making an appearance on my own head prior to the start of Pearls and ‘Heat’ structured management.

BTW, what you are viewing is the result of the surface of the skull, hair follows as a result.

Once the process started, they abated somewhat, although they are still present and not gone.

And yes, that higher rear one was becoming more and more painful until the actual process ‘ignited’ the main Pearl.  But it was worrisome no doubt as I did not have an understanding of it’s nature.  Just had to ride it out.
