How could RED FLAGS NOT HAVE BEEN RAISED ON THE SEVERITY OF THIS PATIENT’S CONDITION !

It’s unacceptable that any medically trained professional would not have raised it somewhere in a report to just have it collecting dust in some electronic drawer or file cabinet.

Now one or two cases maybe acceptable considering the sheer number of patients. Yet the question begs as to whether what’s learned here, translates into better vigilance in terms of not only noting deteriorations, but actually nipping them in the bud before it gets to cases such as this.
